Services: Chronic Care Model

In Hispanic communities, diabetes and cardiovascular disease are grave concerns. Consequently, Mariposa is a participant in the national chronic disease collaborative originated by the National Institute for Health Care Improvement and the Department of Health and Human Services. Mariposa’s physicians and staff work as a team to address the patient’s needs and establish a positive path towards effective management of their condition. By combining clinical care with community education classes and self-management goals, patients are taking positive, progressive steps towards effectively managing their illnesses.
